PHN responsibilities for the TB Program:
• Nurse provides guidance & support to primary providers regarding latent TB
• Regarding active TB, the nurse manages that patient’s care from beginning to end. The nurse receives orders from pulmonologist, infectious disease specialist, etc. for patients with active tuberculosis.
• Case management – education to patients, families, & health care providers in the community & emotional support to patient & family
• Provides care through in-home D.O.T (Directly Observed Therapy) visits for individuals diagnosed with either active or latent tuberculosis
